Appointments
Need to schedule or cancel an appointment? Read here to find out our scheduling guidelines and other helpful information.

To schedule an appointment, call our office at 704-384-9920 between the hours of 7:00 a.m. and 4:00 p.m., Monday through Friday. 

When you need to schedule an appointment, it is very important to call ahead. However, we are aware that emergencies arise. We will attempt to accommodate your urgent medical needs as efficiently as possible. We ask that you arrive 15 minutes prior to your appointment time in order for us to copy your insurance card and update any demographic information.  

Confirmations and Cancellations

Our staff gives reminder calls to each patient 1-2 days prior to their appointment. We do require a 24 hour notice for cancellations. If you are more than 15 minutes late for your appointment, you may be asked to reschedule.

No Showing for an appointment 3 consecutive times may result in a dismissal. New patients, well child exams and adult physicals that no show may not be allowed to reschedule. 

New Patients

Our office welcomes new patients. We advise that you check with your insurance provider to make sure that you are able to come to this office. Most insurance companies provide an 800 number on the member's card and they can assist you with that information. We ask that new patients arrive 30 minutes prior to the scheduled visit. You will be asked to fill out a registration form and a history form. You must bring all insurance cards and co-pay.

What To Bring

There are a few important items you will need to bring with you to your appointment. They are as follows:

  • Your insurance card.
  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency. This will allow your provider to make sure you are on the right combination of medications.
  • A list of any herbal supplements or vitamins you are taking, since any of these can affect your body and its reaction to other medications we may prescribe for you.
  • Pertinent information about your medical and surgical history.
  • Any recent x-rays and, if possible, please have your previous medical records sent to our office prior to your visit. This will aid the physician with your treatment.
