For non-urgent requests, appointments, referrals or to ask simple non-urgent questions, you may contact us at www.mdhub.com.

Office Hours:
Our office hours are Monday-Friday from 8 a.m.-5 p.m.  We are closed weekends and major holidays.

After Hours Support:
We ask that routine questions, prescription refills and appointments be made during regular business hours.  For a life-threatening emergency, call 9-1-1 immediately.  For other urgent needs, call our office at (704) 384-1925.  After normal office hours, calls will be answered by the after-hours triage support team.

New Patients:
We welcome new patients!  We would be delighted to meet your heathcare needs.  On your first visit, we ask you to arrive 30 minutes early to fill out registration and medical history forms.  Please bring your insurance card, any medications you are currently taking and any co-payment or insurance deductible that applies.  Having your previous medical records sent to us prior to your visit will aid the physician with your treatment.

Check-In:
Please bring your insurance card with you for each appointment.  All insurance co-payments and deductibles are due at time of service.  Co-pays will be collected before you are seen.  Please arrive 15 minutes before your scheduled appointment time to allow for the check-in process.

Making and Canceling Appointments:
It is very important to call ahead when scheduling an appointment.  Call us anytime from 8 a.m.-5 p.m. to schedule.  We do require a 24-hour notice for cancellations and it is essential that you arrive on time for your appointment.  You will receive a reminder call 1-2 days prior to your appointment.  Please listen carefully to the information and respond to us if necessary.  Failure to do so may result in your appointment having to be rescheduled.
