Check-In:
It is very important to call ahead and schedule an appointment. However, we are aware that emergencies arise. We will attempt to accommodate your needs as quickly as possible. You may call 704-316-5080 between the hours of 8 am and 5 pm Monday through Friday to schedule your appointment. We do require 24 hours notice to cancel your appointment. It is important to bring your insurance card and a list of your current medications to each appointment and to allow time to copy your insurance cards and update any information in your chart. New patients should arrive 30 minutes before your scheduled appointment.
New Patients:
Our office welcomes new patients. You may call 704-316-5080 between the hours of 8 am and 5 pm Monday through Friday to schedule your new patient appointment. We advise that you check with your insurance provider to make sure that our office is on your insurance plan. Most insurance companies provide an 800 number on the member's card to assist you with that information. We ask that new patients arrive 30 minutes prior to the scheduled visit. You will be asked to complete a registration form and a history form. You must bring all insurance cards.
Office Hours:
8 am to 5 pm Monday through Friday.
After Hours Support:
We do provide an after hours answering service for urgent medical problems. We ask that routine questions, prescription refills, and non-emergency appointments be made during regular business hours. If hospitalization is required, a team of in-hospital specialists will provide your care until you are discharged.
Holidays:
We observe the following holidays: New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving, the day after Thanksgiving, Christmas Eve and Christmas. Should you need us on one of these days, you can call us at 704-316-5080 and an on-call physician will be glad to assist you.
Co-Pay:
All insurance co-payments and deductibles are due at the time of service. If we do not have a contracted arrangement with your insurance plan, you will be expected to pay in full at the time of service.
