Check In:
It is important to call ahead and schedule an appointment. However, we are aware that emergencies arise. We will attempt to accommodate your needs as quickly as possible. You may call 704.384.8680 between the hours of 8:00 am and 5:00 pm Monday through Friday to schedule your appointment. We do require a 24- hour notice to cancel your appointment.

It is important to bring your insurance card and a list of your current medications to each appointment. We ask that your co- pay be paid at the front desk prior to your appointment. Please arrive 10 minutes prior to your scheduled appointment to allow time to copy your insurance cards and update any information in your chart and computer.

New Patients:
Our office welcomes new patients - newborns through seniors and everyone in between!  You may call 704.384.8680 between the hours of 8:00 am and 5:00 pm Monday through Friday to schedule your new patient appointment. We advise that you check with your insurance provider to make sure that you are able to come to this office. Most insurance companies provide an 800 number on the membership card and customer services representatives can assist you in verifying this information.

If you are transferring from another medical practice, please contact that office and request that your medical record be sent to Cotswold Medical Clinic. We strongly encourage patients to schedule a visit before an illness occurs.  This gives you an opportunity to learn about us and for us to get familiar with your medical history. 

We ask that new patients arrive 30 minutes prior to the scheduled visit to allow adequate time to complete registration and medical history forms. Please bring all insurance cards and a list of all medications you are taking or the actual medication bottles.

Office Hours:
Our office is open 8:00 am to 5:00 pm Monday through Friday.

Confirmation and Cancellation:
Our Interactive Voice Response system automatically calls most patients two days prior to their appointment. You may confirm or cancel an appointment on the phone at this time. We do require a 24 hour notice to cancel your appointment.

After-Hours Support:
For a life-threatening emergency, dial 911 immediately. For any other urgent needs, call our office at 704.384.8680. Should you need us after normal office hours, you can call us at this same number and a triage nurse or the physician on-call will be glad to assist you.

Holidays:
We observe the following holidays: New Year's Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving, and Christmas. Should you need us on one of these days, you can call us at 704.384.8680 and a triage nurse or the physician on-call will be glad to assist you. On most holidays, our Urgent Care offices are open from 8:00 am to 8:00 pm. We have three locations:
