How To Reach Us: Phone: 704-384-6901 Fax: 704-384-6902 For non-urgent requests for appointments or referrals or to ask a simple non-urgent question, you may contact us via email through this website by using the Email Us option on the home page. Please allow 48 hours for a response, although most are responded to within 24 hours. Due to HIPPA restrictions, we cannot respond via email. Please make sure you include a current daytime contact number. Office Hours: Our office hours are Monday-Friday from 8:00am-5:00pm. We are closed weekends and major holidays. After Hours Support: We ask that routine questions, prescription refills and appointments be made during regular business hours. For a life threatening emergency, call 911 immediately. For other urgent medical needs when the office is closed, you may call our office at (704)384-6901 and your call be forwarded to the after-hours triage support team. New Patients: We welcome new patients! We would be delighted for you to choose us to deliver your healthcare needs. On your first visit, you will be asked to arrive early to fill our registration and medical history forms. Please bring your insurance card, any medications you are currently taking and any copayment or insurance deductible that applies. Having your previous medical records sent to us prior to your visit will aid the physician with your treatment. Check-In: Please bring your insurance card with you for each appointment. All insurance copayments and deductibles are due at time of service. Copays will be collected before you are seen. It would be helpful to arrive a few minutes before your scheduled appointment time to allow for the check-in process. Making and Canceling Appointments: It is very important to call ahead and schedule an appointment. Call us anytime from 8:00am-5:pm to schedule. Non urgent appointments can also be requested through the Email Us option on the home page. We do require a 24 hour notice to cancel an appointment. It's essential that you arrive on time for your appointment. Failure to do so may result in your appointment having to be rescheduled. Referrals: Our referral coordinators will assist with making appointments for outside tests and specialists. You may contact them directly at (704)384-6907. If your insurance plan requires a referral or pre-authorization, they will obtain it. If it's not required, you may be given information to make your own appointment at a time that is most suitable for you. Please allow us 72 hours when authorization must be obtained. Retroactive referrals are not permitted. We encourage you to be proactive and call your insurance company for coverage guidelines, especially for diagnostic testing (i.e. CT scans, MRI, etc). | ||||||||||||||||||||||||
