About Our Practice

Appointments
Learn about office policies including office hours, check-in, and procedures for new patients as well as scheduling or canceling visits.

Check In:
Appointments are scheduled during regular business hours. We ask that you cancel or reschedule appointments at least 24 hours prior to the appointment time. There may be a cancellation fee if the office is not notified more than 24 hours before the scheduled appointment. To check in for your appointment and complete any required administrative tasks, we ask that you arrive 15 minutes before the actual appointment time.

New Patients:
New patients are mailed a pre-registration form prior to their appointment. We ask that you bring photo ID as well as your insurance card. Having your previous medical records sent to our office prior to your visit will aid the physician with your treatment.

Office Hours:
Our office hours for appointments are Monday - Friday 7:30-5, and the switchboard is open 8-5. We are closed on weekends and major holidays (New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and Christmas Day).

Confirmation and Cancellation:
If you must cancel your appointment, please let us know as soon as possible. There may be a cancellation fee if FCP-Randolph is not notified at least 24 hours before your scheduled appointment. If you receive a confirmation phone call before your appointment, please note the information carefully, and respond to us if necessary.

Missed Appointments:

If you miss an appointment scheduled for you, our policy is listed below:

  • Cancellations should be made at least 24 hours before scheduled appointment.
  • Patients will be notified in writing for first and second missed appointments. The notification will specify information about dismissal for repeated missed appointments.
  • Patients will be dismissed after three missed appointment.
  • Consistent administration of this policy will pertain to all patients for repeated no show appointments.

We feel strongly about the importance of keeping appointments for two reasons.  First, the appointments are scheduled for your care, and we do not want that care to be interrupted.  Second, our schedules are busy, and if you do not keep a scheduled appointment, another patient could have used that time.  Therefore, if you must miss, unless it is an emergency, please let us know 24 hours ahead so we may offer the time to another patient.

After Hours Support:
We do provide an after hours answering service for urgent medical problems. We ask that routine questions, prescription refills, and non-emergency appointments be made during regular business hours. If hospitalization is required, a team of in-hospital specialists will provide your care until you are discharged.

Holidays:
We are closed on New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and Christmas Day.

Co-Pay:
The patient is responsible for his or her co-pay at the time of the services rendered. It will be collected before you see the provider. The co-pay is set by your insurance plan.
