Billing and Insurance
You will find more about billing, billing for tests, insurance plans, Medicare coverage and ABN waivers.

Billing:

All insurance co-payments and deductibles are due at the time of service.  If we do not have a contracted arrangement with your insurance plan, you will be expected to pay in full at the time of service.  Billing questions may be directed to 704-384-1795 weekdays from 8 am to 5 pm.

Billing for Tests:

You may receive a separate bill for your laboratory work.  Depending on your insurance, you may receive a bill from one of the following companies:  Carillion Labs (1-866-546-2991), Presbyterian Lab Services (1-866-411-6958).

Insurance Plans:

The plans below give your family access to the physicians and facilities of Lakeside Family Physicians: Aetna, Alliance, BlueCross BlueShield of North Carolinas, CCN, Cigna HealthCare of North Carolina, CNA/Mail Handlers Benefit Plan, Coventry Health Care of the Carolinas, First Health, HealthCare Savings, HUMANA Network, Kanawha, MAMSI, MedCost, NC Teachers and State Employees, One Health Plan of North Carolina Partners Medicare Choice, Primary Physician Care, Private Healthcare Systems, Inc - PHCS, United Healthcare Insurance Company, United Healthcare of North Carolina and WellPath Community Health Plans.  We also participate with other insurance companies.  Always check with your insurance company before selecting a physician.

Medicare Coverage:

We are a participating provider with Medicare.

ABN Waivers:

Some tests deemed necessary for your care may not be covered by Medicare or your insurance company.  If this is the case, you will be informed of the test and the reasons for performing it.  You will then be asked to sign a waiver stating the possibility of non-coverage was explained and that you may be billed for the test.  ABN waivers are specific for Medicare.
