About Our Practice

Appointments
Learn about office policies including office hours, check-in, and procedures for new patients as well as scheduling or canceling visits.

Check in:
Appointments are scheduled during regular business hours of 8 am to 5 pm. Please call 704-384-0588 to schedule your appointment. Please communicate your needs with the scheduler when you call for an appointment so that we can best schedule your visit.  At the time of the visit, please try to limit your visit to only those concerns for which  you were scheduled.

New Patients:
Our office welcomes new patients, especially family members and children of our established patients. You may call 704-384-0588 between the hours of 8 am and 12 pm and 1 pm to 5 pm, Monday through Friday, to schedule your new patient appointment. We advise that you check with your insurance provider to make sure that our office is on your insurance plan. Most insurance companies provide an 800 number on the member's card to assist you with that information.

We ask that new patients arrive 30 minutes prior to the scheduled visit. You will be asked to complete a registration form and a history form. You may complete these before the visit by going to the "Forms Online" tab in the "About Us" menu tab of our website to download and complete the pre-registration and past medical history froms prior to your appointment. Please bring all insurance cards to your appointment. It will also be very helpful to have your previous medical records sent to our office prior to your visit as this will aid the physician with your treatment. We can fax or send you a Release of Medical Information form for you to send or fax to your past physicians.

Office Hours:
Our office hours are Monday - Friday 8 am to 5 pm. We are closed on weekends and major holidays (New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and Christmas Day).  We close at noon on Christmas Eve and New Years Eve.

Confirmation and Cancellation:
If you must cancel your appointment, please let us know as soon as possible. We strive to offer timely and quality medical care so we appreciate you letting us know if you will not be able to make your appointment.  This will ensure that another patient can have your appointment time.

You will receive a confirmation phone call the day before your appointment.  Please note the information carefully and respond to us immediately if necessary.

Missed Appointments: If you miss an appointment scheduled for you, our policy is listed below:

  • Cancellations should be made at least 24 hours before scheduled appointment.
  • Patients will be notified in writing for first and second missed appointments. The notification will specify information about dismissal for repeated missed appointments.
  • Patients will be dismissed after three missed appointment.
  • Consistent administration of this policy will pertain to all patients for repeated no show appointments.

We feel strongly about the importance of keeping appointments for two reasons. First, the appointments are scheduled for your care, and we do not want your care to be interrupted. Second, our schedules are busy, and if you do not keep a scheduled appointment, another patient could have used that time. Therefore, if you must miss, unless it is an emergency, please let us know 24 hours ahead so we may offer the time to another patient.

After-Hours Support:
For a life-threatening emergency, dial 911 immediately.

We provide an after hours answering service for urgent medical problems. We ask that routine questions, prescription refills, and non-emergency appointments be made during regular business hours. If hospitalization is required, a team of in-hospital specialists will provide your care until you are discharged.  We will make every effort to visit you during your hospitalizatoin to ensure that your needs are being met.

Hoildays:
We are closed on New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day, and Christmas Day. We generally close at noon on Christmas Eve and New Years Eve.  Should you need us on one of these days, you can call our office at 704-384-0588 and you will be connected to the on-call nurse and physician network for further assistance.

Co-pay:
All insurance co-payments and deductibles are due at the time of service. If we do not have a contracted arrangement with your insurance plan, you will be expected to pay in full at the time of service.
