Check-In:

It is very important to call ahead when scheduling an appointment.  However, we are aware that emergencies arise.  We will attempt to accommodate your needs as quickly as possible in emergency situations.  You may call 704-510-8000 between the hours of  7:00 am and 4:45  pm Monday through Friday to schedule your appointment.  We do require 24 hours notice to cancel your appointment.  It is important to bring your insurance card and a list of your current medications to each appointment and to allow the staff time to copy your insurance cards and update any information in your chart. 

New Patients: Our clinic welcomes new patients.  You may call 704-510-8000 between the hours of 7:00 am and 4:45 pm Monday through Friday to schedule your new patient appointment.  We advise that you check with your insurance provider to make sure that our office is on your insurance plan.  Most insurance companies provide a toll free number on the member's card to assist you with that information.  We ask that new patients arrive 15 minutes prior to the scheduled visit.  You will be asked to complete a registration form and a history form.  You must bring all insurance cards. Clinic Hours: 7:00 am to 5:00 pm Monday through Friday. Select office hours on Saturday for established patients only with acute care issues. After Hours Support: We do provide an after hours answering service for urgent medical problems. We ask that routine questions, and non-emergency appointments be made during regular business hours and not after hours.  If hospitalization is required, a team of in-hospital specialists will provide your care until you are discharged. Holidays: We observe the following holidays:  New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving Day and Christmas Day.  Should you need us on one of these days, you can call us at 704-510-8000 and the on-call physician will be glad to assist you.
