Co-Pay                                                                             Insurance co-payments and deductibles are due at the time of service.  If we do not have a contracted arrangeement with your insurance plan, you will be expected to pay in full at the time of service.

Billing
Our billing department will gladly file your claims with your insurance carrier. If payment has not been received within 45 days of filing, you will be notified and asked to contact your insurance company.

Billing for Tests
Labs and other tests performed outside our clinic will be billed by the entity performing the test.

Medicare Coverage
We do accept assignment for and file Medicare for our patients.

ABN Waivers
Some tests deemed necessary for your care may not be covered by Medicare. If this is the case, you will be informed of the test and the reasons for performing it. You will then be asked to sign an ABN waiver stating the possibility of non-coverage was explained and that you may be billed for the tests.
